Eligibility

  • An older veteran in a group setting holding an Army cap, highlighting assistance with daily living needs.

    VA-Enrolled Veterans

    Veterans enrolled in VA health care or referred by Veterans Service Coordinators

    Non-Dishonorable Discharge

    Members from the military, naval, or air service separated under conditions other than dishonorable

    Activated Reserves/Guard

    Current and former members of the Reserves or National Guard, activated under federal orders (excluding training purposes) and who completed the full duration of their active duty call.

    ADL Assistance Needs

    Veterans requiring assistance with activities of daily living (ADLs), such as:

    • Bathing

    • Dressing

    • Grooming

    • Mouth care and oral hygiene

    • Eating assistance

    • Transferring

    • Walking assistance

    • Toileting assistance

    • Incontinence care

Duty Requirements

  • A service duration of 24 continuous months or the complete term of the active duty call.

  • Those discharged for hardship or received an “early out.”

Service Authorization and Hours

Care is authorized by a care coordinator or social worker at a VA Medical Center and paid for by the Veteran’s Administration.

They determine:

  • the type of care provided (homemaker, home health aide (HHA), or respite care), based on the individual needs of the Veteran.

  • The number of hours authorized, on average, ranges from 6-16 hours per week.

  • Additional hours of private-pay care are available, if desired.

How to Apply for VA In-Home Care if You’re Already Enrolled in VA Health Care:

  • 1. Contact Care Coordinator

    Contact your care coordinator, if you already have one.

  • 2. Schedule VA PCP Visit

    If you don’t have a care coordinator, make an appointment with a VA primary care physician (PCP) and undergo an assessment to discuss your in-home care needs.

  • 3. Referral for Services

    If a care need is identified, the PCP will send a referral to your VA caseworker for further evaluation.

If you’re not already enrolled in VA health care:

Complete a VA Form 10-10EZ application to enroll in VA health care.

To access this form, go to va.gov/forms and type 10-10EZ into the search bar.

That gives you a choice of two ways to complete the application:

  1. Complete the 10-10EZ application online and submit it to the VA.

  2. Download a 10-10EZ application PDF, complete it, and mail it to the VA.

Note: Choose the method you prefer. Only one application needed.
